CV Joint query...

Hi all,
When you know your van....you know when somethings not quite right!!??
I can hear a (as best to describe) a high frequency 'grinding' noise from the transmission / cv joints etc when I am travelling in a straight line (after about 1m of driving from cold at about 45-50mph) and hear it especially well when turning while putting the clutch in to come to a stop (ie turning into the drive) happens on both locks.
I cant isolate where it is actually coming from. Any ideas.
If it is the cv's is it best to replace inner and outer while you are doing it?
Also....the new Syncro Services web site seems to have changed so much that most parts are now not listed!!??
Brickwerks have all CV's except front outer (as its a 16" van).
Any help gratefully recieved.
