Ready for a dumb question? When I test my batteries they show a suitable reading but when I test engine running the meter sits at zero and does nothing. Do I need to change the settings?

Are you measuring the voltage, and where are you trying to measure it? Measuring from the positive terminal of your battery to the earth terminal should read 12.6 volts with the engine off and a bit higher 13-14v with the engine running. Just check your multimeter is on the right scale, normally 20v DC.

I get the readings I'd expect off starter and leisure but then when the engine's running and revving there's no reading at all. The starter gives over 12.5 the leisure is about 11 which is why I wanted to check its charging.

You may be overcharging and the reading is going off the scale, try the next voltage scale up.
