I got them from........http://www.kinkymicks.com/" onclick="window.open(this.href);return false; - he's fine, really.
Doesn't seem to be listing them but If you give him a call I'm sure he sort you out.
They are 23mm thick with a centre boss of 80mm diameter so ideally you should get 'boss adapters' machined to suit.
You will also need to reduce the height (about 2-3mm) of the wheel nuts you use to attach adapter to disc/brake drum.
And a set of studs (Brickwerks).

I've been using thesefor a couple of years now - no problems.
I thinks it's about £100-120 for a set of four plus studs (have a look at Brickwerks) then boss adapters - do you know of a local engineering outfit?
If you're after 16in rims for your T4 have you looked at the Sharan - there are a number of steel rims ( PNs 7M3601027E and 7M0610027J), probably work out cheaper to buy a new set of five than adapters+studs+ boss adapters +T5 rims.
Not sure of the boss diameter of these rims compared with the T4 so there may be a bit of fiddling needed.
