Try this morning routine ....

Check oil and coolant before opening driver's door
Open door, get in, press accelerator pedal straight to floor, thus 'setting' the automatic choke ... gently lift foot from pedal. Place in neutral (you did leave the van parked in gear after all ... didn't you ?)
Insert ignition key, release steering lock and turn key, engine should start immediately and rev to about 1500 rpm.
When stabilised gently tap the accelerator pedal to 'knock' the choke down to 1/2 setting.
Once warmed up the choke will 'drop off' completely and settle the rpm at ~750-800

This is the idiot's guide to the 1.9 auto-choke ... works equally well with the 2.1 on DG carb.

So you havent done the FULL service and tune up that was suggested to you some time ago??

You have a 25 year old vehicle that inevitably has some wear and tear, it also does ot have any electronic engine management and is running a carburetor.. any dirt or missadjustment will make things worse, but you are expecting a 25 year old vehicle to drive just like the 5 year old stuff you are used to...
As a result the idle wont be perfectly stable, then you have the slack in the drivetrain which will make the vehicle shunt which causes the engine to load and unload.. thus giving a lumpy ride.. I dare bet on a slight incline so the transmission is loaded it will be relatively smooth...

Mr Hulley has the procedure.. follow that.. and give it a bloody good service
