Could be a pinhole leak in the fuel line. It draws in air when higher fuel demand but allows normal(ish) flow at lower revs. Have you had the fuel filters cleaned lately? Blocked filters could also cause those symptoms.
If it is a fuel related issue, then a systematic check of the various fuel components is required, starting from the tank, or the carb.

Re: cutting out problem
I had similar issues with cutting out which got progressively worse until my van would not tick over at all once up to working temp. It started from cold fine and I could keep it going with a foot on the gas but it ran pretty rough. It turned out to be the vacuum hose on the carb, worth a check.

Re: cutting out problem
I'm going over all pipework this week, and I did notice the fuel pipe into the carb is rock hard so will replace that first so probably a bit of luck the van packed up .
