Big brakes upgrade. Rear disc conversion.

Evening All
My T25 is taking shape very nicely. It'll soon be ready for its first outing.
I'm probably going to annoy a few people with my take on the VW van/camper scene. I'm modifying my T25 in an 80's/90's style which includes a lower stance, 18" wheels and hopefully bigger brakes as well as a rear disc conversion.
My question this evening is which is the simplest conversion? I want better and bigger discs on the front preferably vented. Is this possible by fitting from another VW? If not is there a reasonable kit? Is the same available for a rear conversion. The van is fitted with an AAZ. I'm not dumb and understand basic mechanics but will not be attempting any type of brake conversion myself.
