Help! A bit smokey and lumpy in 5th

Hi, My truck smokes a bit and is lumpy in 5th gear? She starts fine and drives around town with no problems but as soon as she goes into 5th on the motorway she feels lumpy and struggles doing 50 on small inclines. Can anyone point me in the right direction or check list to resolving this?

Good point Fritz. She's an 89 (F) Doka 1.6 TD with the JX Engine code. All I can see that is missing from the engine is the Breather pipe that connects the Rocker cover, inlet manifold and the air intake.
