when i did mine i just pulled the old one out and pushed the new back thru the fixed tube to the front then passed the sleeved bit around the gear box to the engine bay bulkhead hole then connected up, the only fiddly bit was connecting the peddle end up you need to be a bit of a contortionist with gynecological skills but if you can raise the front up on ramps or a hoist I would think this would make the job a bit easier.
Nasty nasty job if some bugger has pulled the fixed tube off and slung it over a proverbial hedge prior to your ownership.
The pedal end connection isn't that bad sheesh, lie on your back with your head under the van and your feet out the driver's side in front of the wheel, sit up a bit and it will all be very clear to you.
