Maybe a bolt sheared first and was rattling around, lots of stress then on the cap and other bolt.

Photos will tell all, especially the fracture faces of the cap and the bolts.
PS bearing cap bolts arent subject to shear, they are axially loaded so its all about crack propagation, usually initiated at the thread root/runout which is a stress raiser.
Unusual with production rolled threads though, as opposed to cut threads.
My gut says bearing cap fatigue i.e. AAZ may have limited operating hours before failure.
