It might be worth taking a look at the engine/gearbox mountings.....

First off.. it was towed back? if there was a problem with the final drive in the box, then it would have done a lot of damage at some point..

SO, I would check out the CV joints as a start..prob not them but with a rythmic sound in relation to road speed, its a pretty good start.. even worth swapping the right drive shaft for the left but fitting the right wheel cv to the left gearbox output (and the right gearbox output to the left wheel) and the left wheel cv to the right gearbox output etc.. This will reverse the forces in the CV joints so may clunk on over run (but not as likely) its a temporary fix though.. eventually you will need to buy a new set..

Check the gearbox oil level.. better still, drain and refill, collect the old oil in a clean tub and instect for debris.. it could be that the synchro rings have started to break up.. If they have, then driving it will end in disaster.. and unless you are a gearbox expert, leave the rebuild to a specialist (Shiplakes!)

I'm afraid it sounds like some of the teeth have stripped from 4th gear, wait for the experts but it sounds terminal.

Rayt25,,,,,,,,the problem we have now is you have left some/most of us in suspense, to enable us to sleep at night we need you or your good buddy to strip the offending box and let us know what exactly was the origional fault, just incase we suffer the same symptoms in the near future.
