Vibration - what are the causes?

Driving around 40 - 45mph in 4th gear the van vibrates slightly, (enough for the rear view mirror vision to become blured). What causes this? I always put it down to the engine being over-revved and coming out of balance at some point in its life. Someone else said it could be worn engine mounts?
Any ideas?

i doubt even if you have a diesel nonturbo 40 in 4th is over reving!

Sorry by over revving I mean that the engine has been over revved before and is now damaged slightly and somehow unbalanced? When I was a lot younger and used to drive my mum and dad's Ford Escort, I used to thrash it within an inch of its life regularly going over 6,000 rpm I suspect, (afterall it was a 1.3 so needed plenty of encouragement..). As a result however, the engine sounded rougher and rougher every time I used it. Vibrations also started to creep in.
Obviously I haven't thrashed the van, but the partner uses it sometimes and is...... well let's just say she is less mechanically sympathetic than me....

If it happens at a specific speed, I'd say get the wheels balanced.

CHECK YOUR TYRES FOR EGGS/DAMAGE ALSO!!
Tyre balance as mentioned can cause this but also tyre damage will do it too

Wheel bearings, gearbox, CV joints all places you can get rumblings from. Get someone in the back to pinpoint where its coming from.
