1.6td cam belt trying to throw it's self off

Firstly hello to every one on this forum I have gleaned lots of useful information to help keep my 1990 1.6td california on the road, thank you.
I am now after some help with a problem. my timing/cam belt is trying to throw its self off its pulleys (succeded once) it is fraying on the outer edge as the tensioner trys to hold it on!
The problem started when the injection timing was ajusted (by a Diesel specialist!) by turning the pump a few degrees, they insisted this could not cause the problem and something else had gone ie the tensioner, so new belt tensioner etc, etc, fitted, still problems, so back to mechanic more bits replaced checked supposed to ok untill....... the belt gives up a few miles from glastonbury and destroys the engine, engine has now been rebuilt by the engine shop and the injector pump and injectors rebuilt by a specialist, everything is now back together rebuilt engine, pump, new tensioner, new belt etc, etc,.......the belt is still trying to come off......
