engine stalls?

hi, my van periodically has a few mintues of stalling when i change gears, its normally as im slowing down or stopping when i dip the clutch, if i hold the accelerator down more when gear changing it wont stall either, but you try doing that when using the brakes . . . Whens it running it will tick over fine, only seems to stall when driving and changing gears.
very annoying feels like im constantly bump starting me van, any ideas.
DG 1.9 petrol engine btw

Is it mainly when hot or cold? when she does idle properly does she sound smooth? It could just be that the idle speed is a little too low, I adjusted mine up a tad earlier today as although she never stalled she sounded a little lumpy and was on the verge of stalling. Other things to check would be the vacume pipe coming from the back of the carburetor to the side of the distriputor, if this is split or has come off then this will make her run rough.Also always worth checking distriputor cap/rotor arm and plugs for wear ,sometimes if everything is a little worn it can all add up to a potential problem. yep, everything. new rotor arm dizzy cap, HT's. plugs/Oil/water/air filter last month.
the rubbers on the pipes comin from the carb are worn pretty bad, but i thought if it ticked over fine they wouldnt be an issue? i wanna change them anyway if any1 can point me in the right direction where to get em, GSF said NO ! Tried me idle speed adjuster, but that doesnt seem to speed/slow the engine, maybe fooked ?

Don't know the carb, but this problem is typically the idle (or slow running jet) on solex carbs. Might be worth a try if one is fitted.
