The fridge will get warm running on 12v or 240v and the area between the van side and back of fridge will be prone to condensation...if you do not vent it then the condensation may cause rust/mildew probs! ... so best to vent if you can...above fridge level if poss! :D

Had a Talbot Autosleeper and the fridge was raised a couple of inches above the floor with a grille under and on top of the fridge. The ventilation path was thus internal. Under the fridge, round the back and back over the top into the van. The fridge had an external flue for the gas option but no external air vents.

Just fitted mine as AngeloEvs describes.
I fitted a set of thermo fan behind my top vent to help with airflow.

There is a thread on here with some picture

If you don't vent the fridge to the outside air, you will find the van will get very warm inside, the hotter the ambient temperature, the hotter the air coming off the back of the fridge.
