Could either be a sticky throttle cable or perhaps your choke isnt opening up causing your engine to run to rich when warmed up. Get a pal to start and rev the engine and see if your throttle is returning ok. Also have a look at your choke flap when warmed up to see if it has opened up.

It's supposed to rev high if it's cold.
If the fast idle mechanism is working ok, the engine will idle at 2500 rpm until the throttle is blipped.
The correct way of starting the DG with pierburg 2e3 is to depress the accelerator pedal once fully.
Remove your foot from the pedal and turn the key.
the engine should idle at a fast rpm.
As the temp of the engine rises, the idle speed comes down automatically. 1500, 1200. finally 950 rpm
All this takes about 6-8 mins

Has this just started happening? Has anybody touched anything to do with the carb in the recent past?

No its not with it been cold it can start up fine and then after driving down road it starts revving fast and I mean really fast. No one has touched carb my wife uses van as a daily ride and it has just started happening when it happens if you turn van off and turn it over again it's fine and then happens again or sometimes it doesn't .

Ok, well you need to get the duct off and watch the butterflies move.
It's a long shot, but the second throttle is operated by vacuum, this might be sticking on.
I would be removing the carb and making sure this mechanism is working without issue
