Have you shimmed the flywheel/driveplate properly?
Does the crank have a groove in it?

Have you checked/set the crank endfloat with a Dail Test Indicator?

You should always check it if you have had it in pieces.
How far have you stripped it? have you got the thrust washers in the crank if the case has been split?
With the engine in the van can you test for endfloat on the crank pulley?

Oil in water or water in oil?
could be as simple as a oil cooler? Which engine? 2.1? Does it have a oil cooler?
