just looked there are no makers marks or numbers on it im afraid and the box went in to the bin ages ago
But ebay is full of similar ones.

This exactly the type of bracket that I mounted. I leave the TV on the bracket full time, the only thing that seems to get loose is the swing arm. Mine is tightened up with a allen wrench.
Not where I purchased, but has the specs.
