Feet to the front, head in the tailgate for me, diagonally if I'm on my own. My van also has a spotlight in the back so I can still read with my head at that end.

Slept on the floor of our uninsulated, draughty panel van (some windows with dodgy seals!) one night last August. It was cramped but with 2 duvets and plenty of pillows (plus copious gallons of Guinness) I slept through.
Missus woke up though!
In the AM I arose to a nice bacon butty and a wonderful view of the hills around Glossop.
About to complete our R&R conversion on the replacement Caravelle.
Now I vote 'feet to front'. For the reasons below:
1. Tailgate is an ideal headboard.
2. I can view the roof mount DVD screen nicely.
3. I can't stand having my feet squashed.
4. Missus likes to be poised for loo action.
5. I'm a big bloke so I prefer the weight over the engine bay
